Personal item is not the same as overhead baggage. The free piece normally goes under the seat. A bag that physically fits an overhead size is not included unless your fare, add-on, seat or status provides that entitlement.

Airlines for Europe (A4E) describes 40 × 30 × 15 cm as a guaranteed minimum personal-item size across its member networks. Airlines may allow more. It is not a universal maximum and should not be confused with a second free overhead trolley.
